Please read the section below for instructions to provide Proof of Age, Proof of DC Residency, and DC Health Forms.

For students that are new to any DCPS school, a proof of age document is required to enroll your student. Please refer to the list below:


Proof of Age Options

Provide 1 (one) document:

  1. Birth Certificate
  2. Hospital Records
  3. Previous School Records
  4. Passport
  5. Baptismal Certificate


All students are required to prove District residency every school year. Families have several options to meet this requirement including, having their residency verified by the school official through an interagency sharing process, completing an OTR application, or providing proof of DC residency from the options below. For questions regarding residency, contact your school enrollment official.


Please Note:

1.       Name of Enrolling Person must match the name on all proof of residency provided.

2.       Address of Enrolling Person must match the address on all proof of residency provided.


Proof of Residency Options

Provide 1 (one) document:

  1. Pay Stub
  2. DC Government Financial Assistance
  3. Certified Dc Tax Form-D40
  4. Military Housing Orders
  5. Embassy Letter

OR

Provide 2 (two) documents:

  1. DC Motor Vehicle Registration
  2. DC Driver’s License/Non-Driver ID
  3. Lease with Payment
  4. Utility Bill with Payment


All students attending DCPS must present proof of appropriately spaced immunizations by the first day of school. Please complete and return your student’s school health forms including the Universal Health Certificate with Proof of Immunizations, and Oral Health Assessment. Health forms are essential to ensuring appropriate medical care for your child during the school day.
