2.Please download the portfolio template by clicking here.

3.Please Complete EVERYTHING on the template. Save as a Powerpoint (part of MS Office Suite) file. If you don’t have Powerpoint, you can open this file and use it in Google Slides (a completely free version of Powerpoint – just google and download to use) – but you must still save it as a powerpoint file – not a PDF file. Please name this powerpoint file with your First and Last name

4.Upload the completed template using the upload feature in the next step below "Upload Completed Template"

5.Please complete the design honorarium agreement here. (It will open a seperate form to complete)

6. Please complete the W9 form here. (it will open a seperate form to complete)

DGS WILL NOT BE ACCEPTING HARD COPY OR EMAIL FORMS ANY LONGER.
